General Dentist  
Requirements: 
Graduate of an accredited dental school, DDS, DMD or DD required. Bi-lingual in English and Spanish preferred. Valid Indiana Dentist Licensing, valid Indiana CSR Licensing, and valid Federal DEA licensing required. Minimum of two years of practice experience as a General Dentist.
Educate individuals in the nature of oral hygiene, oral health-related conditions, and oral health preventative services. Examine individuals requesting care, diagnose their dental/oral conditions, develop a clinically indicated treatment plan and render dental care services based on the developed treatment plan. Perform quality clinical and radiographic evaluations to diagnose and treat diseases of the oral cavity.
Appropriately administer dental anesthesia to limit the amount of pain experienced by patients during procedures. Repair decayed and broken teeth by the placement of various restoration techniques and materials, and when necessary, extracts teeth. Communicate with dental staff regarding patient treatment plans. Prescribe medications as necessary. Documents and follow treatment plans that include appropriate preventive and restorative therapy that has been reviewed with the patient.
Utilizes Electronic Dental Record System (OCHIN/EPIC Wisdom) to document all clinically indicated patient care services and information. Participates in community dental health initiatives. Adhere to all federal and government regulations, including the state dental board rules and regulations, and HIPAA and OSHA regulations.
